How to Collect Client Signatures via SMS


You will first have to create a new client in order to collect their signature. If you are unsure of this process please check out the help article on How to Add a Client.

1. During the process of creating a client, you will be given the option to choose how your client will be signing the authorization. You will then select My client will electronically sign by SMS." 

2. Type the phone number in the bottom field that says "Contact Phone".

3. Click the purple "Send SMS" button.

4. Kindly advise your client that they will be receiving a text from Medchart. This text will ask them for their signature and will include a link that will redirect them to a window requesting for their electronic signature.

Please note that the text may take 5 minutes to 1 hour to be received by your client depending on the service or communications carrier of your client.

5. Please instruct your client to complete the following steps.

Once the link has been clicked; they will be directed to a window that states the following:
Please provide Electronic Authorization for Medchart Legal Proceedings Inc. to request records.

a. Click the purple button that states "Provide Signature"

b. A window will appear requesting for a signature within the window

c. When your client is satisfied with their signature, click "Done"

d. Scroll and continue below to see "Provide Initials"; click "Provide Initials"

e. A window will appear requesting for initials within the window

f. If satisfied with your initials, click "Done"

g. Click the terms and conditions stated below. This disclaimer will allow and give your attorney authorization to affix a copy of your signature and initials on your behalf to requests for medical records related to their representation of you

h. Click "Accept"

i. Once "Accept" has been clicked, you will receive the following message, "Electronic Signature – Completed"

6. You will receive an email notification to inform you that your client has signed an authorization and it is now ready for use on Medchart.
